A West Virginia MAT Clinic Hired 3 Prescribers. X-Waiver Changes and Enrollment Errors Blocked Billing for 9 Weeks.

THE SITUATION
This MAT clinic had operated for 4 years under a single physician. In 2024, they expanded hiring one additional MD and two APRNs to prescribe buprenorphine for OUD treatment.
All three had completed their training requirements. 9 weeks after their start dates, every buprenorphine-related claim from the three new prescribers was denied.
The billing team was using the same enrollment checklist they’d used in 2021 before the DEA eliminated the X-waiver requirement in December 2022 and restructured MAT prescriber credentialing rules.
WHAT WAS ACTUALLY WRONG
DEA registration process had changed post-X-waiver:
After the SUPPORT Act changes and the 2022 X-waiver elimination, MAT prescribers no longer need a separate DEA X-waiver but they must notify DEA of their intent to prescribe buprenorphine for OUD through a standard DEA registration update. The billing team was looking for X-waiver numbers that no longer exist. Two of the three prescribers hadn’t completed the updated DEA notification process.
WV Medicaid OUD provider enrollment has a separate SUD treatment provider certification:
WV Medicaid requires MAT providers to hold a separate OUD treatment provider certification through WV DHHR before the claims process. This is separate from standard Medicaid enrollment. None of the three new prescribers had this certification.
APRN prescribing authority wasn’t confirmed at the payer level:
Highmark WV and Aetna both require APRNs to submit prescribing authority documentation separate from their NP licensure before the MAT claims process. This documentation hadn’t been included in either APRN’s enrollment packet.
WHAT WE DID
Days 1–3: Confirmed both APRNs and the MD had completed DEA buprenorphine notification update. One MD had not submitted the DEA online notification the same day.
Days 3–7: Submitted WV DHHR OUD treatment provider certification applications for all three prescribers. Flagged for expedited review citing active patient panel.
Days 7–14: Submitted APRN prescribing authority documentation to Highmark WV and Aetna for both APRNs. Resubmitted enrollment packets with correct documentation.
Day 18: WV Medicaid OUD certification approved for all three. Highmark and Aetna approved APRN enrollments the same week.
Retroactive: WV Medicaid allowed retroactive claims to certification date. Recovered 8 weeks of MAT claims $52,000.
